Replacement certificates can be ordered by sending a check made payable to The George Washington University in the amount of $40 to the Office of Graduate Medical Education, 2300 Eye Street, Suite 707, Washington, DC 20037. Enclose a statement with your name, phone number, specialty and years of residency training along with the address to which the certificate should be mailed. Please allow 4-6 weeks for delivery.
