TIPS
* Study Closures:
An IRB approved study can be closed in the OHR when all data collection and subject interaction has been completed, all private identifiable information has been de-indentified, and all codes/links /keys to the identifiable information has been destroyed. This is applicable to all Expedited and Full Board studies. Please submit the Closure Form when these criteria have been met. (not applicable for Exempt studies)
* Study Renewals:
Please submit a study Renewal Form within 30 days prior to the study expiration date. If your renewal has not been submitted or received IRB approval by that date, all enrollment and study procedures must stop until IRB approval has been obtained.
* Changes to IRB approved studies:
All studies are required to have IRB approval prior to the implementation of any changes. Please submit the completed Modification Form, and include all applicable documents that are being modified.
REMINDERS
* Please use only the most current OHR submission forms! Do not download website forms until you are ready to use them. Forms are often updated to assist with the review process; older forms may not provide the information required for review, and could result in a delay in approval.
* When deciding if your research will require IRB review and oversight, we have a worksheet that can help guide you in your decision process. Please refer to the “Human Subject Research Determination Worksheet”, on the forms page.
* All Principal Investigators and key members of the research team are required to have completed CITI training prior to IRB review and conducting human subjects research at GW. This training is required to be kept current until your study is closed through this office.
* Investigators will be assigned an IRB number upon OHR receipt of new study submissions. You may request a number prior to submission if required by an external institution, by emailing your request to: ohrirb@gwumc.edu. Please include the name of the principal investigator, title of the research project/study protocol, and sponsor (if applicable).
