Instructions for Copyright Clearance Request

To assist faculty members in obtaining permission to reproduce copyrighted materials for print and electronic course reserves, Himmelfarb Library’s Reserves Department has registered with the Copyright Clearance Center (CCC). The CCC provides a centralized system for obtaining permission to use materials for academic reserves.  Using this service allows us to help you in obtaining copyright clearance easily so you can legally place your materials on electronic or print course reserve.

Please fill out the form for each of the articles/book chapters that you are requesting. You can submit more than 1 request per form.

Follow all instructions below to ensure that your request is processed in a timely manner.

Instructions for filling out the CCR form

  1. Please fill out all the required fields for each item that you are requesting permission for. It is essential that you supply accurate information.  Forms with incorrect citation information will result in delays.
  2. Please include the Banner Department Code (sometimes call an “Alias”) for your department (for billing purposes). This is required.  If you don’t know this code, contact your department administrator.  You will receive an email confirming that we have received your request(s).
  3. We will input the information on the CCC website and will contact you promptly regarding the status of the request. The CCC may grant or reject permission to use the materials.
  4. Permission is usually granted by the CCC within 1-3 days, but may take longer, depending on the material being used.  Therefore, we cannot guarantee you will get permission quickly.
  5. If permission is granted, we will inform you of the fees being charged by the CCC (which may vary depending on the material and number of copies being requested).  At that time, you can instruct us to place the order, or cancel it.  If you instruct us to place the order, your department will be billed for the fees.
  6. Please note: There is a $3.00 processing fee per request that is charged by the CCC, regardless of whether the CCC grants permission, or we place the order or cancel it.  This fee will be billed to your department.
