Introducing Online Request Forms
As of July 15th, 2002, the Himmelfarb Library ILL and Document
Delivery Department will no longer be accepting paper, fax
or email requests for materials. All requests must be made
through the online ordering module accessible by following
the link to "Other Request Forms" on the library
web page. (http://www.gwumc.edu/library/services/docdelivery/ill_service.cfm)
See below for step-by-step instructions on how to use our
online forms.

Instructions for Entering Online ILL Requests
Step 1

- Type your first and last name.
- Type your email address.
- Select your affiliation from the drop down list.
- Click on next.
Step 2
- Select the type of request from
the drop down list. Your options
are:
ILL - Book,
ILL Chapter from a Book
ILL Journal Article
Offsite Storage Book
Offsite Storage Journal Article
Photocopy Chapter from a Book Photocopy -
Journal Article
- Click on next.
Step 3

- Select your preferred Delivery Method. Your options
are:
Pick up
Email
Mail
Fax
- The purpose of your request will default to Research.
The other options available are Patient Care and Education.
- Under Service Option, please indicate how quickly
you need the material and then indicate whether you
would like us to borrow an item from outside our collection
if we do not own the title.
- Under Billing Options, select how you would like
to be billed for the transaction.
- Please note: Authorization for billing to the MFA
is required for all transactions. Please be sure to
print off your confirmation email, obtain authorization
from your Department Head the authorization and fax
us the authorized request.
- Enter your request information in the appropriate
boxes.
Verify that the name and contact information shown
at the bottom of the page is correct.
- Click on next.

Step 4
- Enter your mailing address, phone and fax information.
- Click on submit. You will get the confirmation screen
shown below.

- To submit additional requests, simply click on the
Make another request button. You will be prompted
to repeat steps 2 and 3, but you will not need to
re-enter any contact information.
