Military Program Health Science Laboratory Technology Associate of Science Degree Program
Please wait until you have completed the entire certificate program before submitting your application for admission to the A.S. degree.
Students must have completed the certificate program with a 2.5 GPA in
order to apply for the A.S.
Application:
To apply to the A.S. program, use the Apply Online Now link. Create an application account and then fill out the “Application for Admission.” Do not fill out the “Application for Contract MLT Program;” this is not the correct application. Applicants to the HSLT A.S. program are not required to complete the statement of purpose or submit letters of recommendations. Additionally, the application fee is waived since these applicants are all prior students of the university.
Transcripts:
Once you have submitted the application, transcripts can be sent the Military Education Specialist at the following address:

Please do not send transcripts to the GWU Undergraduate Admissions Office as they may not reach the A.S. program office or may be significantly delayed.
Upon receiving your application and all transcripts demonstrating completion of the required 12 credit hours, the admissions office will review your application. If all requirements are satisfied, you will receive an admissions packet in the mail with instructions and forms on fulfilling the Continuous Enrollment requirements and submitting the Application for Graduation.
Do’s and Don’ts in Preparing Your Online Application for A.S. Candidacy in Health Science Laboratory Science.
- DO submit official transcripts of all college work in original, sealed envelopes.
- DO submit a Change of Name form to the University Registrar if your name has changed from the name on file in the certificate program. The form can be found here: http://www.gwu.edu/~regweb/web-content/onlineforms/changeofname.pdf.
- DON'T send official transcripts or have official transcripts sent before submitting your application. If you have already done so, please attach a note to your application indicating that the transcripts should be on file.
- DON'T send payment with your application for admission. You will be billed later for continuous enrollment and graduation, as appropriate.
