Patient Navigation and Survivorship LISTSERV:

In response to feedback from the navigation and survivorship communities we have created a Navigation and Survivorship LISTSERV, to use as a communication tool for individuals and groups who are vested in patient navigation and cancer survivorship issues.

If you wish to be added to this list please send an email to casnp@gwumc.edu with the word "listserv" in the subject line and your First and Last name in the body of the email. You will receive email messages when they are sent out from other members of the group, and will be able to send email messages to the group.

If you have any questions, email casnp@gwumc.edu. We hope you will find this to be a useful way to troubleshoot issues that arise and collectively advance the fields of patient navigation and cancer survivorship.

    This site provides an overview of the concept of Patient Navigation and links to resources and valuable information that will aid the efforts of anyone participating in navigation programs. This site also offers insights into the work of people like you who are committed to improving patients' experiences in the health care system.

  • Colorado Patient Navigator Training: http://patientnavigatortraining.org
    This site offers a full curriculum designed to build patient navigator skills and knowledge. Courses and workshops relate to issues navigators deal with every day and are taught by patient navigation experts.

  • Resource Guide to Patient Navigation: http://www.fccc.edu/
    Whether you are considering starting up a patient navigation program at your institution or you are in the midst of implementing your program, there are materials in this guide that you can use to adopt or refine best practices in your program.

  • Resource Guide to Health Literacy: http://www.fccc.edu/
    This guide is meant to assist those interested in health literacy understand how it impacts healthcare, health outcomes, patient safety and, ultimately, the public's health.

  • ACCC's Cancer Care Patient Navigation: A Call to Action online resource: http://accc-cancer.org/education/education-patientnavigation.asp
    The Association of Community Cancer Centers' Center for Provider Education provides has developed an online resource titled Cancer Care Patient Navigation: A Call to Action. ACCC launched this project to:
    • Identify barriers to access to care that patient navigation can address
    • Increase successful implementation of patient navigation services
    • Refine staffing models
    • Establish effective metrics for measuring patient navigation services internally and for benchmarking patient navigation services against other community cancer centers.

  • The GWCI Survivor's Map: http://www.gwumc.edu/survivorsmap/
    Cancer survivors may have physical, psychological, social, spiritual and economic late effects of cancer treatment. The Cancer Survivorship Map provides information and resources to assist adult survivors of pediatric cancer in the transition out of active cancer treatment.
